This notice applies to treatment records we keep at all good dose studios. The website contact form isn’t part of your health record, so please don’t send medical details through it.
How we use your information
We use your health information to treat you, to bill for your care, and to run our practice. We share it beyond that only when the law allows it, or with your written permission.
Marketing and photos
We never use your information or your photos in marketing without your separate written permission. You can withdraw that permission at any time.
Your rights
You can ask to see or get a copy of your record, ask us to correct it, ask us to limit what we share, ask for a list of certain disclosures, and ask us to contact you in a particular way.
